FAST PREDICTION OF INFARCT OUTCOME IN ACUTE STROKE USING DIFFUSION MRI

TECHNICAL DESCRIPTION

The present invention is a technique that predicts the final infarct growth from the acute stage of ischemic stroke. This technique was developed and evaluated on a series of about 100 patients and is based on an automatic image analysis approach of the Apparent Diffusion Coefficient (ADC) maps obtained on a conventional 1.5T magnetic resonance (MR) scanner. The underlying model of the image analysis builds on previous findings from retrospective clinical imaging studies that reported on a mild decrease of the regional ADC value in the ischemic penumbra (Oppenheim et al. 2001). This latter is a pertinent indicator of the final infarcted brain areas.
Results indicate that this technique is at least as sensitive and specific than the concurrent prediction approach that computes the perfusion-diffusion mismatch index. However, the ADC-only prediction does not necessitate the injection of a contrast agent (as required by perfusion MR) and relies on standardized ADC computation from conventional diffusion-weighted MRI sequences. The acquisition and image analysis are fast (40 secs and 5 mins respectively), with minimal intervention from the operator.
